These can b used as a cover or with. Microfiber inserts that snap in with a "sleeve" that protects baby's bum & inserts. They all come with inserts and sleeve. I prepped them but never used. In my others I prefer CBI Charcoal Bamboo Inserts. They r pricey but if u get involved with coops on Facebook u can get them reasonable and unlike microfiber can b up against baby's skin.. hope that helps

They're called is one size fits all ---- newborn to about 35lbs. They have snaps that adjust according to baby as he/ she grows. I hope that answered or question.
