Just stumbled upon this and that I could clear up what grading is. It is a process of examining a card for any kind of flaw be it factory made or due to it being handled and based on what is found, giving it a number from 1-10 (higher is better). Yes it does cost but you will be able to say a card is PSA 8 and this DOES increase a card's price (especially if the card is a rare/hard to get card). Flaws can include improper centering of a card, improper cutting of a card, and so on.

I don't know really. I think it tells what condition it's in and you gotta pay for it but if you get a high score it makes the card more valuable. The term gem mint floats around the grading industry
