To me CTO means cancel to order. Most countries have either in the past or even now will cancel a stamp before it leaves the post office. They don't care if you throw it away, used the stamp or keep it 100 years. That stamp is paid for and cancel. In USA always a stamp is only cancel when used, past and now. This is the way I look at it. If you have another way of looking at it Please let us know.
