It is prepay. Straight talk is through Walmart so you can purchase reload cards there or online. I will post another picture soon. It takes a memory card not a Sim card.

It will depend on location. The package will be light so it shouldn't cost much but it will vary on location. I would think it would be around $5 but I will have to compare rates.

Yes it's in good condition. I will double check for minor flaws and post if any when I get home but I don't think there's any.

It looks like there are some very minor surface scratches probably from being in my purse when I used it. Nothing noticeable unless in direct light
