Takes 4 AAA Batteries
slot for memory card. I don't have anything that goes with it (cords, manual, and whatever else that is supposed to come with it). The camera does work and you could get along just fine without the things that are missing.

It does have a display screen on the back. I'm sorry I can't do free shipping at the moment, I just don't have the extra funds to spend on it right now.

I would say it's about 1 inch thick, 3-4 inches wide, 2-3 inches tall. I'm not for sure on this as I don't have the camera with me at the moment (I'm at work).

Janiefay, I forgot to answer your question about the make and model number. I think that is included in the the name of the camera as I didn't see any other names or numbers on the camera anywhere. I don't know much about this stuff, sorry I'm not able to answer your question any better.
