Coordinator Instructions: Sheets must be submitted with order and banking form and postmarked by 2/14/14. Total value of completed bonus sheet is 100 points and is not inclusive of individual UPS point value. Incomplete or partial forms will not be accepted. Forms postmarked late or with incorrect UPC's will not be accepted.
