Does it come with the extra foam blocks that were originally included in the package?
What kind of condition is it in (Any rips, tears, holes, malfunction, smells, etc)?

To the best of my knowledge it does work. The item was given to me and I personally never used it. The way you see it is the same as when I received it. I don't smell any odors from the mask. No additional foam blocks came with it, as it didn't come with original packaging. I hope I answered all your questions.
