Not sure when the coupon was issued, but I'm assuming this can be redeemed at any Chick-fil-A by just presenting it. I've had different coupons where it has had lines that need to be completed but the establishment took the coupon without them filled in. However, I am not responsible if you cannot find a store that will take the coupon without the fields filled in. =P
