The obverse features Karol Waclaw Swierczewski in the center, coat of arms to the lower right. Swierczewski was born on February 22, 1897 in Warsaw, and died on March 28, 1947 at Jablonki, near Baligrod. The reverse shows 'The Order of the Cross of Grunwald', a military decoration created in November 1943. There are several signature varieites for this note.
