The listing, XFX Radeon HD 6770 video graphics card, READ DESCRIPTION has ended.

This is a working card, it just needs the fan replaced. It was clogged with dust and the fan bearings were going out so it would start to heat up and the computer would shut down to protect it.

Very sure on that measurement. From fan blade edge to edge is exactly 65 millimeters.

No it does not come with it's own power supply. I ran it for 2 years fine with an internal 850 watt power supply.

Looking up parts for it it should have 3 screws.
