dose it have the thing that goes in the usb port on a media center pc or do you have to buy the rf adapter

The only thing in the package is the remote, batteries, and the instruction manual. if your Media Center PC accepts infrared commands then this remote will work, otherwise you would need to buy a USB-IR adapter.

I honestly don't know. The manual is in the package but the package is sealed. You could scan through all the preprogrammed codes to find one that works. If you have the original remote for the Picturetel, then this remote can also learn the codes...
