It's in the original plastic case which is in good shape. The disk has no apparent scratches.

Thanks for checking it out. I'm sorry, but I don't have any way to download pictures to my computer. The case and disk are in very good condition, There are a few minor wear scratches on the plastic case, but it is original. The disk has a little scratch around the outer edge from where it was used, but it isn't really noticeable unless you look very closely. I actually didn't even see them when I looked it over before I posted it.

The code should be on the printed material on the case, it's the serial number code.....
