I bought this at the Ed Hardy store in Hawaii a little while back. Wore it only once or twice. You won't find many like this at all. There were only 2 in the store I bought it from, and I paid close to $200 for it. It is a button up, and has Ed Hardy embroidered onto the back. Has a lot of tattoo artwork, skulls, pirate, wolf, etc. Very cool shirt, I just have no real use for it any more. It is a Medium, and fits me pretty snug around the chest. It does not have a place for collar stays, so I would consider it "loose" collar. It is lightweight fabric, so perfect as a summer-time long sleeve.
