I don't know the code but if u go to Verizon they can master reset the phone without a problem and I believe it's free. I had to reset it to clear all my info off of it

No but you can use it as a prepaid or a plan through Verizon u just have to have it set up which ever way u want it.

You can use it for a plan or prepaid you will have to bring it to a veriZon store though
