I no longer have the packaging for this but it has never been used. I had it in my car for my verizon droid x phone but have a different phone now so can't use it.

it has a suction cup that you can mount onto the windshield or a sticky pad for the dash. i have never actually used it though. I am pretty sure I paid $40 for it from verizon when i bought my phone. you might be able to research it at the verizon website for more details although it does have the instructions
