this is a pretty cool thing. if you go i believe on beechnut's website they have a link that goes to a printable of this form. it basically is you save your upc codes from beechnut products to mail in and then get coupons off. i have 38 upc codes for beechnut baby food in the jar. and i will send ya a form too. i started saving the upc codes when my daughter was eating the baby food but now shes onto solids and real foods so if anyone can use em ill be happy to send to ya :) ill send to winner as soon as i can after auction,
